Job Description: Position: OUTREACH WORKER Location: Council on Aging Date Opened: 7/8/24 Date Closed: 7/26/24 (or until filled) Position Type: Part-Time (15 hours/week) Salary: $20.00/hour (grant-funded) Brockton's Council on Aging (COA) is seeking a dynamic part-time Outreach Worker. This critical position is responsible for assisting elders and their families in defining needs, facilitating access to services and providing information and referrals by conducting assessments, determining and reviewing case management plans, coordinating with other agencies, implementing delivery of services, and performing other related duties. The mission of the COA is to provide easy access to an array of general programs, information and resources, and socialization opportunities for seniors. The successful candidate will have the opportunity to work with a dedicated group of employees and volunteers committed to serving seniors and their families. DETAILED JOB DESCRIPTION Statement of Duties: The Outreach Worker is responsible for assisting elders and their families in defining needs, facilitating access to services and providing information and referrals. The employee is required to perform all similar or related duties. Essential Functions: Conducts assessments of clients, determines and reviews case management plan, coordinates with other agencies and implements delivery of services and provides telephone reassurance to isolated frail elders. Makes home visits, assists seniors in defining their needs, facilitates access to services, makes appropriate referrals and provides assistance to clients. Coordinates with local agencies and public safety departments to respond to, alleviate and resolve crises. Assists elders with Medicare and other government or private health and welfare related programs, especially during Medicare's annual open enrollment. Actively seeks new clients; solicits and accepts referrals from private individuals, social service agencies, religious and community organizations and public safety agencies. Compiles and maintains all necessary program reports and records; submits monthly statistical report to Council on Aging Director. Maintains varied confidential client files and records. Provides feedback to the Director of the Council on Aging regarding clients' needs. Assists in developing programs to benefit the elder population and the community. Assists with and works with Brockton Housing Authority to resolve housing issues. Assists with housing applications, fuel assistance applications and SNAP applications. Attends workshops, seminars and lectures to develop and enhance knowledge in the field of gerontology. Assists members at front counter and over the phone when needed. Provides SHINE services once certified (successful candidate will be trained). Supervision Required: Under the general supervision of the Council of Aging Director, the employee is familiar with the work routine and uses initiative in carrying out recurring assignments independently with specific instruction. The supervisor provides additional, specific instruction for new, difficult or unusual assignments, including suggested work methods. Supervisory Responsibility: The employee, as a regular part of the job, is not required to supervise any City employees. Confidentiality: The employee has regular access to confidential information such as department records that potentially contain health and other sensitive information. Accountability: Consequences of errors, missed deadlines or poor judgment could result in monetary loss and adverse public relations. Judgment: Numerous standardized practices, procedures, or general instructions govern the work and, in some cases, may require additional interpretation. Judgment is needed to locate, select and
