SUPERVISORY RESPONSIBILITIES None. SUMMARY Under the general direction of the Project Coordinator, the Facilitator/Health Educator will assist with the day-to-day programmatic and administrative responsibilities of the Health and Dental Services Program. Assist in the fulfillment of the agency's vision, "For Those We Serve," by empowering individuals who thrive as healthy, self-sufficient and contributing members of our communities. DUTIES AND RESPONSIBILITIES/ESSENTIAL FUNCTIONS Maintain case history records and prepare reports. Interview clients individually, in families, or in groups, assessing their situations, capabilities, and problems to determine what services are required to meet their needs. Serve as liaisons between students, homes, schools, family services, child guidance clinics, courts, protective services, doctors, and other contacts to help children who face problems, such as disabilities, abuse, or poverty. Develop and review service plans in consultation with clients and perform follow-ups assessing the quantity and quality of services provided. Address legal issues, such as child abuse and discipline, assist with hearings and providing testimony to inform custody arrangements. Counsel parents with child rearing problems, interviewing the child and family to determine whether further action is required. Consult with parents, teachers, and other school personnel to determine causes of problems, such as truancy and misbehavior, and to implement solutions. Arrange for medical, psychiatric, and other tests that may disclose causes of difficulties and indicate remedial measures. Refer clients to community resources for services, such as job placement, debt counseling, legal aid, housing, medical treatment, or financial assistance, and provide concrete information, such as where to go and how to apply. Counsel individuals, groups, families, or communities regarding issues including mental health, poverty, unemployment, substance abuse, physical abuse, rehabilitation, social adjustment, child care, or medical care. Provide, find, or arrange for support services, such as child care, homemaker service, prenatal care, substance abuse treatment, job training, counseling, or parenting classes to prevent more serious problems from developing. Collect supplementary information needed to assist client, such as employment records, medical records, or school reports. Provides a safe and healthy work environment. Assists with maintaining and monitoring the project's day-to-day operations including but not limited to: managing an activities calendar including room reservations for project meetings/sessions; coordinating related logistical tasks; corresponding with partnering medical providers' staff; managing budgets and expenditures; and ensuring proper documentation and file maintenance for project participants. Develops, delivers and performs health education and outreach activities in youth-targeted areas. Builds, fosters, and maintains relationships with staff, partners, and youth participants. Facilitates health education sessions as needed by working in partnership with clients, school districts, medical providers and youth service agencies to deliver curriculum as scheduled. Actively promotes and encourages participation of all participants and creates a safe and inviting environment for clients. Maintains effective and ongoing communication with the Project Coordinator. Assists with coordinating care and works closely with staff and Project Coordinator to ensure health education curriculum care services are integrated and coordinated with other services. Increases community awareness of Health Services and Education through attendance at stakeholders' meetings and conferences, events. Works with Fresno EOC Communications department to provide awareness via publicity, presentations to local media and other media relations activities. Provides progress and related report required for program implementation. Monitors and collects data for evaluation and performance measures. Reviews outreach education materials and curriculum. Administers surveys; local and state pre and posttests. Maintains confidentiality of records and information on participants. Attends and participates in scheduled required staff meetings/trainings/workshops. Travels for agency business using personal transportation. Maintains regular attendance and punctuality. Works evenings and weekends as required. Performs other duties as assigned. QUALIFICATIONS To execute this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION: Associates or Vocational Degree in Social Work, Counseling, Nursing or related field required. Bachelor's Degree in Social Work, Counseling, Health Education or related field preferred. EXPERIENCE: Minimum three (3) years work-related experience in public health or teaching required. Minimum three (3) years of experience in effective group facilitation and creating successful connections with participants required. OTHER QUALIFICATIONS: Highly trained and knowledgeable in areas including, but not limited to youth learning process and Fresno County and Local Resources. Ability to work well with a diverse population, and develop and sustain collaborative relationships with contracted service providers and representatives of partnering agencies. Ability to maintain confidentiality. Culturally competent. Able to promote and sustain diversity, inclusion, equity and respect for parents, children, staff, and community partners. Understanding of early childhood care and education, early intervention and child development trends. Ability to assist with interpreting, analyzing and utilizing data sets to inform service coordination and scheduling. Capable of exercising good judgment in a variety of situations. Strong administrative and organizational skills. Ability to maintain a realistic balance among multiple priorities. Strong customer service skills. Self-motivated and able to work independently. Ability to speak in a clear, informative, and engaging manner to small and large groups. Highly resourceful team player. Ability to identify issues proactively and address before issues escalate. Bilingual English/Spanish or other secondary language preferred based on the needs of the clients and the program. Proficient in Microsoft Office Suite and related business software. Effective presentation, written and verbal communication skills. Willing to work non-traditional hours and days to meet the needs of this position. Maintain a valid California driver's license, reliable transportation, adequate auto insurance as required by State law, and insurability by agency carrier for those driving agency vehicles. Pass required background investigation TB clearance, acceptable criminal record statement, Department of Justice fingerprint clearance, and child abuse index clearance.
