Responsibilities To help ensure optimal nutritional status for all residents and patients through nutrition screening, care planning and education. ESSENTIAL FUNCTIONS * Performs nutrition screening through review of the medical record, diet history, meal rounds, and interview with resident/patient and family according to department policies and accepted standards of care. * Establishes realistic, measurable nutrition goals and develops individualized care plan for the low nutrition risk resident. Evaluates progress towards goal and documents interventions in a timely manner and in compliance with departmental policies and accepted standards of care. * Provides appropriate diet education to resident/patient and/or family as needed in both the nursing home and home care setting. * Assists with the development of nutrition care and dietary policies and procedures and monitors adherence to policies. * Assists with the development of regular and modified diets and menus. * Assists with staff education as assigned. * Represents department at Team and Care Plan conferences, Resident Council meetings, and other committees as assigned. * Assists with the maintenance of data base in Geri-Menu computer system. Requirements * Completion of an ADA approved Dietetic Technician Program. Registration by the Commission on Dietetic Registration * Minimum of two years of experience
