Opening Full Time 40 hours Range URG 103 OBGYN Admin-SMH Responsibilities POSITION SUMMARY: Provide support to the birth certificate office in interviewing moms and entering data into the NYS database ESSENTIAL DUTIES: Preparing Interview Forms 40% * Retrieve birth notices * Check worksheets and see what information is needed for completion * Log book entry Interviews 40% * Verify worksheet information and fill in missing information * For each single mom, explain the Acknowledgment of Paternity * Explain various forms and get required signatures Chart Abstraction and Data Base Entry 20% * Obtain information from patient charts and prepare enter birth certificate data from worksheets into the NYSDOH web site Miscellaneous 10% * Must have a pleasant attitude, must be sensitive to difficult situations and work cooperatively with others in the office and hospital staff * Weekend coverage (4 hrs.) to be rotated equally among staff * Routine filing and coping of forms, letters, etc. * Answer numerous phone calls from new mother's regarding birth certificates and Social Security numbers QUALIFICATIONS: * 2 years of post High School courses with emphasis in Data Processing or Medical Terminology * Accreditation as Medical Record Technician * 1-2 years related work experience; or equivalent combination of education and experience.
