#LI-SA1 Summary/Objective: This dual role involves overseeing essential functions of the bereavement and volunteer programs supporting Hospice. The coordinator is responsible for coordinating and supporting bereavement services, facilitating grief support activities, and maintaining communication with bereaved families. Additionally, the role includes recruiting, training, coordinating, and supervising hospice volunteers. Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Support and Outreach: * Assist in identifying and reaching out to bereaved families who may benefit from grief support services. * Provide initial contact and follow-up phone calls to bereaved individuals to offer emotional support and assess their needs. * Refer families and individuals to appropriate grief support resources and services. Coordination of Bereavement Services: * Oversee bereavement assessment and bereavement care planning. * Develop and maintain a list of additional community resources to meet the needs of moderate and high-risk bereaved individuals. * Provide education on bereavement for staff during orientation and ongoing training. * Help schedule and organize individual and group bereavement counseling sessions. * Assist in planning and coordinating memorial services, support groups, and remembrance events. * Maintain accurate records of bereavement activities, including contacts, services provided, and feedback from families. Administrative Support: * Assist with the preparation and distribution of bereavement literature, newsletters, and other supportive materials. * Maintain a database of bereaved families, ensuring information is up-to-date and handled confidentially. * Help with the collection and analysis of data related to bereavement services for reporting purposes. Volunteer Coordination: * Develop, implement, and document strategies to recruit hospice volunteers from the community. * Assist in onboarding, training, and supporting volunteers, providing guidance, and monitoring their activities. * Coordinate volunteer schedules and assignments to ensure adequate support for the hospice program. Community Engagement: * Help promote hospice bereavement services within the community. * Participate in outreach events to raise awareness about hospice care and the importance of bereavement support. * Collaborate with community organizations, religious institutions, and other stakeholders to enhance bereavement support. * Conduct outreach and informational sessions to educate the public about volunteer opportunities within hospice care. Work Environment/Physical Requirements: The employee must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements. The employee may experience changes in work conditions related to weather issues, such as traveling in rain, snow, etc., and must be able to adapt to the work environment to meet patient care needs. Required Education and Experience: * Compassionate, empathetic, and a good listener with a passion for serving those experiencing grief. * Excellent interpersonal and communication skills, both verbal and written. * Strong organizational and time management skills with attention to detail. * Ability to work independently or as part of a team. * Proficiency in basic computer applications, including email and database management. * Understanding of and respect for diverse cultural and religious practices related to death and grief. * Commitment to maintaining confidentiality and professionalism in all interactions. Preferred Education and Experience: * Graduate from an accredited institution of social work (preferred). * Minimum of 2 years of hospice experience or education in grief or loss counseling (preferred). Skills, Knowledge, and Abilities: * Must be able to travel to the place of service to perform visits as assigned. * Able to read, write, speak, and understand English. * Able to relate positively, effectively, and appropriately with residents, families, community members, volunteers, and other center staff. * Meets all health requirements, as required by law. * Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
