To achieve this unwavering support, Familylinks provides a wide range of family-centered services in the areas of Behavioral Health, Service Coordination, Youth and Family, and Aging Services. The Community Ambassador will serve in a Community Health Worker inspired role and will conduct street outreach and one-on-one contact with older adults ages 50+ residing in the Hill District, Homewood, and the greater East End neighborhoods of Pittsburgh as part of the Wise Neighbor Project. This position will work directly with senior adults to connect them to community resources that will help them improve housing stability, food access, transportation access, social connection, and health care utilization. The Community Ambassador role is a professional position working in the development and coordination of aging services, public health education, health promotion, and community outreach activities. This position supports aging in place and positively influences social determinants of health. The successful candidate must possess the sensitivity, awareness, and skills associated with working with individuals of all ages and backgrounds. Extras of joining our team: Insurance * Monthly allowance towards medical insurance * Free vision insurance with the option for additional coverage * Free dental insurance * Flexible Spending Account (FSA) * Employer sponsored life insurance * Employee Assistance Program (EAP) * Pet Insurance Professional * Three weeks of PTO in your first year * Eight paid holidays * Employee referral bonus program * 403(b) retirement plan matching contributions * Tuition reimbursement and educational stipend * Opportunities for career advancement * Public Service Student Loan Forgiveness (PSLF) Essential Duties & Responsibilities: * Conduct outreach by making phone calls and visiting residential homes to engage with older adults ages 50+. * Identify challenges that older adults face with living at home as they grow older, help them understand the resources available to help, and connect them to community-based support. * Foster trusting relationships as the face of the team and the first point of contact to assist older adults ages 50+ in meeting their health and human service needs, identifying new needs, and coordinating care. * Provide basic information on health resources, financial budgeting, housing support, food insecurity, or other relevant resources. * Advocate for the older adults' community, medical, and behavioral health needs. * Empower clients to self-advocate and follow through with treatment (making and attending scheduled appointments, setting goals, and community networking). * Work closely with the Wise Neighbor Team to connect clients to the program and to community resources. * Participate in ongoing training and increase knowledge in the areas of community health work, aging services, social determinants of health, and healthcare. * Enter and maintain participant records in an electronic database and assist with compiling data to prepare records and reports. * May work some evenings and weekends with advance notice. * Maintain electronic outreach tracking forms and prepare reports when necessary. Requirements * High School Diploma or equivalent required, some college course in social work, public/business administration, education, health or related field preferred. * Two or more years of community engagement experience is preferred. * Community Health Worker certified training will be provided if the candidate does not already have this certification. * Working knowledge about the strengths, needs and barriers for older adults age 50+. * Knowledge of community resources for appropriate referrals or willingness to learn. * Ability to work well with people of diverse social, cultural and economic backgrounds. * Working knowledge of computers and Microsoft Office Suite, including Outlook, Teams, Excel, and Word * Excellent communication and organizational skills, along with ability to set priorities and schedule time efficiently. * Valid driver's license * Self-directed and flexible to meet the needs of program participants. Working Conditions * Employee can expect to spend at least 60% of their time working in the field and in-person inside the homes of older adults age 50+ and the remainder at Vintage Center for Active Adults. * Must provide their own transportation. * In-person interactions and conversations are core to The Wise Neighbor Project's success. This position is based at the Vintage location. Extensive local travel will be required within a 5-mile radius of Vintage. At times it may be necessary to travel to other Familylinks locations for training and meetings. Mileage reimbursement is provided. * Lifting up to 10 lbs. maximum and occasionally carrying such articles as files, ledgers, food boxes and small tools. * A large amount of walking and standing may be necessary in carrying out duties. * This position is grant-funded, and therefore, it is limited in time to the duration of the grant. * This position allows for a semi-flexible schedule due the need for meeting with identified individuals around their schedule.
