Key Responsibilities: * Provide comprehensive, integrated services that are culturally competent and trauma-informed. * Advocate for clients within our community and NorthCare, ensuring they receive the support needed for recovery and independent living. * Conduct health promotion activities, including vital checks, health education, and lifestyle interventions. * Collaborate with a multidisciplinary team to support behavior change and treatment goals. * Maintain accurate clinical records and meet productivity expectations outlined by management.
