Social Worker III- (Therapist) | The Pavilion at Williamsburg Place | Williamsburg, Virginia About the Job: $10,000 Sign on bonus offered! The Social Worker III plans, directs, defines and coordinates individualized and group therapeutic support and services to patients with behavioral problems, psychiatric disorders, post-traumatic stress disorder, substance abuse and physical and emotional abuse. Conducts individual and group therapy, crisis intervention, case management, and education. Roles and Responsibilities: * Conducts psychosocial assessment of patient and develop an effective treatment plan that applies therapeutic principles appropriately while ensuring patient safety at all times. * Coordinates and plans programs and activities to meet patient's social and emotional needs. * Promotes recovery, symptom reduction and the achievement of the highest level of social functioning. * Provides crisis intervention and assist families in understanding the implications and complexities of a life situation and its impact on lifestyle. * Participates in multidisciplinary treatment team meeting to discuss psychosocial assessment, treatment plan and ongoing discharge plan. * Communicates or aids family members in understanding and supporting the patient. * Manages a caseload and maintain accurate and timely documentation to assure patient records comply with facility and accreditation standards. * Facilitates education, process groups, support groups and referrals as requested. * Obtains information on the patient's needs, skills, interests and limitations through patient observation and patient/family interview for the development and implementation of the treatment plan. * Identifies, assesses and coordinates patient cases and anticipates their needs related to illness, hospitalization, finances, education and home/community services. * Promotes recovery, symptom reduction and the achievement of the highest level of social functioning. * Conducts intakes and assists in coordinating placements within facility programs as required. * Supervises social work interns completing field placement, as required. * Provides direction and guidance to social services staff or other staff members, as required. * Develops and executes clinically relevant activities in coordination with activity professionals, as required. Education/Requirements: * Master's degree in Social Work from an accredited college or university required. * Three or more years' related experience working with comparative patient population required. * State licensure, as required by the state in which the facility operates; which could include Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Social Worker (LSW).
