Summary: Under the direction of the Sexual Health Clinic Coordinator, the Health Educator will assess individual and group health needs in the Sexual Health Clinic and in community settings; maintain up to date knowledge of STI/HIV transmission, treatment, disease, PrEP, and perform pre and post test counseling. Conduct Rapid HIV test and provide STI/HIV education, prevention methods, and risk reduction strategies to patients of the Sexual Health Clinic (SHC). Identify patients who are eligible for PrEP and referral them to initiate PrEP. Additionally, provide linkage to care for patients who test HIV positive. Develop and provide patient level education in other related topics (i.e. medication adherence, hypertension, diabetes, and Hepatitis A, B, & C. Participate in all staff meetings and staff development trainings. Essential Functions: * Provide pre and post test counseling and prevention education at The Ruth M. Rothstein CORE Center, various outreach events, and within the Cook County Health system. * Perform rapid HIV testing through finger stick. * Collect and enter data into the appropriate databases and spreadsheets. * Track all Health Education activities on the SC Tracker Spreadsheet. * Identify and enroll patients who are HIV negative into the PrEP program. * Complete the appropriate patient assistance application (Gilead, Viiv, and/or PrEP4IL) for new and current PrEP patients. * Documents all patient encounters, phone calls, and outreach attempts into the electronic medical record and in CAREWare. * Provide excellent customer service when interacting with patients and internal/external customers. Qualifications and Skills required for the job: * Bachelor's degree in a health-related field, public health, health education, psychology, social science, or a minimum of four years of counseling service required. * CDPH or IDPH HIV Testing & Counseling certification. * Must be able to work well in a fast-paced environment and meet deadlines. * Must have basic knowledge of Windows, MS Office/PCs. * Ability to work independently and in a team setting. * Must work non-traditional work hours including nights or weekends at the request of supervisor. * Must possess good communication skills (verbal and written). * Demonstrate cultural competence working with diverse populations, including: MSM, those recently released from incarceration, sex workers, individuals of extremely low income, substance users and/or those in recovery, and homeless individuals. * Must be able to stand and sit for prolonged periods and be able to lift 10 lb boxes occasionally, frequent walking, and occasional bending. * Must adhere to all policies and procedures of CCHIP (Cook County HIV Integrated Programs), The Ruth M. Rothstein CORE Center, and Cook County Health. * Adheres to all applicable State, local, and Federal laws.
