23-24 Social Services Resource Navigator Join Sixteenth Street Community Health Centers (SSCHC), a federally qualified community health center in the heart of Milwaukee's southside. Member Duties : Resource Navigator AmeriCorps Members within SSCHC's Social Services Department will increase capacity to assess and provide supportive resources to individuals to address their Social Determinants of Health (SDoH) needs. Resource Navigators will also have opportunities to create or develop projects or events applicable to the Social Services department. Activities include: Apply SDoH screening to patients during Primary Care visits in clinic setting. Assist patients reporting needs to connect them to internal or community resources. Educate patients about self-advocacy, show them how to research and access resources needed when facing critical life situations. Refer patients presenting with moderate or high-level needs to other service providers as appropriate. Conduct survey with patients for behavior change or intent to change behavior to improve health.
