Requirements Saint Alphonsus needs a Locums Cardiothoracic Surgeon to cover occasional weekend call and occasional PTO coverage! start: ASAPSpecific dates TBDSchedule: 7am-7amEMR: Epic1 CT Surgery dedicated OR4 available PAs rotate to support surgeon in ORPatient Population: Adult/GeriatricRequired Procedures: Adult cardiac surgery General thoracic surgery Aortic dissection CABG Tube thoracostomy Pericardial window Thoracotomy lung resections and pneumonectomy Heart valve repair/replacement surgery Dissection VA VV ECMO placement. Bill Rates needed for Submission: Malpractice must be included in the ratesOrientation24 Hour On Call - cap of 4000/24 hours to include at least 3 hours gratisHoliday 24 Hour On Call 1.5xCall back - cap of 450/hour to apply after gratis hoursHoliday Call Back 1.5xConversion fee 25k rate cap Travel - 750/round trip cap to include all taxes and feesLodging - 179/night cap for hotels to include all taxes and fees. Preferred Lodging: Candlewood Suites Boise – Towne Square an IHG Hotel.Rental Car - To be reimbursed by facility not to be used with personal car/mileage reimbursement. 50/day cap including all taxes and fees regardless of car class. SUV/upgrades over cap will need to be preapproved will only be approved if needed for inclement weather.Mileage - IRS Standard Rate to be reimbursed by facility only to be used with personal vehicle not to be used with rental car.Please make sure you have the following items readily available for your provider at the time of acceptance in order to expedite the credentialing process MD diploma Internship/residency/fellowship certs All medical licenses DEA Board Certification Life support cards- BLS Current and previous certificates of insurance Driver’s license Professional color photo/headshot CMEs for the past 2 years Immunization records including TB Flu HepB Tdap Varicella and COVID References Case logs *Please Note: Trinity Health does not reimburse for any application or credentialing fees.*
